This change migrates all the relevant files and updates made
to the dvb/demux framework, required for mpq demux driver.
The snapshot is taken as of msm-4.4,
'commit a74dd0fdc772 ("mmc: core: retune after un-gating the clocks")'
In addition, introduce a few code changes to reduce checkpatch
warnings, typos and other style issues.

There are several anonymous enums here, used via a typedef.
Well, we don't like typedefs on Kernel, so let's de-anonimize
those enums. Then, latter, we may be able to get rid of the
typedefs, at least from Kernelspace.

"enum dmx_ts_pes" and "typedef enum dmx_pes_type_t" are just the
same enum declared twice, since Kernel (2.6.12). There's no reason
to duplicate it there, and sparse complains about that:
drivers/media/dvb-core/dmxdev.c:600:55: warning: mixing different enum types
So, remove the internal define, keeping just the external one.
Internally, use only "enum dmx_ts_pes", as it is too late to drop
dmx_pes_type_t from the userspace API.
